1.  Print out & Fill out Water Service Application

2.  Email us the following Information: (Which ever applies to you) smidwater@pacbell.net

     (a) Completed & Signed Water Application (Owner or Tenant)

     (b) Proof of Ownership or Tenant Lease Agreement (ex: Title Deed or Grant Deed. You can obtain one at the L A County Registrar-Recorder/County Clerk Office.)

     (c) Copy of your California Drivers License/ California Identification Card
